The writing task changes at discharge
You have already documented the visits. At discharge, the report has to bring the course of treatment together without repeating every appointment. That is a different job from another daily SOAP note.
A useful starting point is your summary of where the patient began, what changed, and where they are now. An AI scribe can turn that spoken summary into report text, so the work begins with a draft instead of an empty discharge document.
An episode summary in your own words
Consider a fictional therapist dictation:
"At the first visit she stopped on the landing at work because of right knee discomfort. Her goal was to use the stairs without stopping. At discharge she reports taking the same stairs without a rest on most days. She still uses the rail. We discussed continuing the agreed home programme. She understands how to contact the service if she wants a further review. The treatment goal has been met according to her report."
An illustrative discharge passage could read:
"At the start of treatment, right knee discomfort interrupted stair use at work and required a rest on the landing. At discharge, the patient reports completing the same stairs without a rest on most days, while continuing to use the rail. The therapist records the treatment goal as met on the basis of the patient's report. Continuing the agreed home programme and access to further review were discussed."
The draft connects the beginning and end of treatment. It does not need to reproduce the visit history to make that comparison readable. The status and future arrangements were supplied in the therapist's summary.
Choose the document for this point in treatment
Astralcyte includes episode discharge summary and interval progress report formats. The progress report is available when treatment is continuing; the discharge summary gives you a starting document for the end of the episode.
These reports are drafted from the account you provide in one recording. Your recap can draw together the relevant history you have already reviewed. That lets you use the app for the writing task while the original visits remain in your existing record.
Finish the report without uploading the history
Edit the draft and copy it into the patient record. Your recording, transcript, and report stay in encrypted storage on your device. They are not uploaded to Astralcyte, as described in its privacy policy.
