The format changes what the draft needs to do

A chart entry records the care and response. A handoff brings the current situation and the next team's task into view. You may be writing about the same event, but copying one document into the other still leaves work.

Choosing the document before drafting gives the software a more useful job than "summarize this." Astralcyte includes nursing note formats and SBAR, so your account can become the kind of text you need.

Two illustrative drafts from the same event

Consider this fictional summary. The nurse supplies the request as well as the events:

"The patient reported discomfort in the chair. I helped her reposition and added back support. She said the discomfort eased. At handoff I want the incoming nurse to reassess her comfort after she returns to the chair. She is comfortable in bed now."

A short DAR entry could read:

Data: Patient reported discomfort while sitting in the chair.

Action: Assisted repositioning and added back support.

Response: Patient reported eased discomfort afterward.

An illustrative SBAR draft could read:

Situation: Patient is comfortable in bed now; reported discomfort earlier while sitting in the chair.

Background: Repositioning and added back support eased the reported discomfort.

Assessment: Patient reports improved comfort after the action.

Recommendation: Incoming nurse to reassess comfort after the patient returns to the chair, as requested at handoff.

The second version gives prominence to the request the nurse stated. It is not a new recommendation from the software. The difference is the purpose and arrangement of the text.

Buy help for the writing you actually do

If your main task is charting, DAR, DARP, focus charting, SOAPIE, or PIE may be the useful choice. If you also spend time composing handoffs, SBAR provides another drafting option in the same app. Edit the result and copy it into your record or communication system.

Astralcyte makes these drafts on your device. The recording, transcript, and text stay in encrypted storage, without going to the company. The privacy policy covers both kinds of document.
